Complete the
online
Family Registration form
to the right. All families must complete
this online form every year to ensure that
we have your latest contact information and
medical history for your children. Summer
camp registration sessions have been added
to the family online registration form.
Choose the session in which you would like
your child to attend. Please include both
first and second choices. |

Download,
print and complete all necessary
medical and waiver forms and send them
into the KCCFA office. The following forms
must be completed:

Physician Evaluation form (for
children who have received any
cancer treatment within the last 18
months) |

Medication
Registration form |
|
|
PRN form |
|
|
KCCFA
Camp Waiver form |
|
Once you have completed all forms, we will email you
a camp confirmation package containing bus times, an
activity list, a packing list and contact numbers.
If you do not have an email address or do not have
access to a computer, your confirmation package will
be sent to you in the mail.
Please note that KCCFA uses a
priority system for camp programs
When spaces are limited for camps and programs,
KCCFA gives priority to children and families who
are recently diagnosed, recently bereaved, or who
have never been to camp before. |
